SRIOV: LDOM Fcinfo Intermittently Shows HBA State "Unknown" on QLogic Controllers Connected to Tape Devices (Doc ID 2718449.1)

Symptoms

This issue started  after we upgraded 11.4.
It was working fine on 11.3 and the same issue happens on almost all 11.4 servers.

We have observed this issue on multiple servers QLogic controllers connected with tape drives.

We observed that QLogic HBA controllers connected to Tape drives, fcinfo intermittently shows "unknown" state after some days,
and it becomes online after reboot . But it will reoccur after some time. Or if I am rebooting any CDOM/IODOM, the respecive
HBA from the domain goes off and become unavailable.
