bdadiskutility fails with "Error code is 12/Device at[Enclosure : Slot] is not Disk"

Symptoms

Configuring a disk after replacement with bdadiskutility fails with:
Error code is 12 .
Device at [<ENCLOSURE> : <SLOT>] is not Disk
Device at [<ENCLOSURE> : <SLOT>] is not Disk
1. Running bdacheckhw reports that the model string for the disk in the slot being replaced is completely empty. Output looks like:
ERROR: Wrong disk <SLOT> model :
INFO: Expected disk <SLOT> model : <MODEL_NUMBER> . <MODEL_NUMBER> .
INFO: Expected disk <SLOT> model : <MODEL_NUMBER> . <MODEL_NUMBER> .
2. Running lsscsi shows that there is no disk in the slot being replaced.
3. MegaCli64 output also finds not disk at the expected slot:
