Symptoms

Disabling LIO service OR pulling both LIO cables causes pool fault on peer head and IO data corruption

This can trigger a system panic.

The panic string will be:

Pool '[zpool name]' has encountered an uncorrectable I/O failure and the failure mode property for this pool is set to panic.

The panic stack will include:

zio_suspend
zio_done
zio_execute
