Symptoms

FC lose connectivity to devices.

See following errors:

kernel: lpfc 0000:86:00.1: 1:0338 IOCB wait timeout error - no wake response Data x3c
kernel: lpfc 0000:86:00.0: 0:0338 IOCB wait timeout error - no wake response Data x3c

Changes

Server firmware recently been updated:

Thu Sep 30 11:07:17 2021 System Log critical
upgrade to version 5.0.2.24 succeeded
