HSM: Cannot Stage In Files, scsi_init_pkt() failure

Symptoms
Customer stated that due to a hardware issue with the previous server, they needed to re-install HSM on new server and storage hardware.
Oracle HSM starts fine with all the tape hardware recognized successfully.
However when they try to stage/read in any file, the tape is loaded, the device log shows it reads 1MB and then it stops.
The /var/opt/SUNWsamfs/trace/sam-stagerd trace file showed the following entries:

Customer moved off of a Dell PowerEdge R730, with Intel Xeon E5-2637v3, using 2x dual-port QLogic QLE2662 16Gb fibre channel adapters, running Solaris 11.3 with Oracle HSM in version 6.1-04...to a new configuration on a Dell PowerEdge R7515, with AMD EPYC 7262 using 2x dual-port Emulex LightPulse LPe31002-M6-D 2-Port 16Gb fibre channel adapters, running Solaris 11.4.50.126.3 with Oracle HSM 6.1-06 (6.1.67).
