Exalytics Guest VM Fails With Md Device Error "/dev/mdp1 does not exist (Doc ID 3021000.1)

Symptoms

After the restart of Guest Vserver, Unable to ping and ssh the server.
While doing so getting the below error:
Error: Device 2684 (vbd) could not be connected. /dev/mdp1 does not exist.

Changes

Restarted the server
