Exadata Database Machine Discovery Fails: ibnetdiscover Returns Status 1

Symptoms
While trying to discover an Exadata Database Machine cluster (this is an X2-8 with 14 cel nodes and 2 db nodes) after providing the IB switch name and nm2user password, discovery finds everything except the rest of the IB switches and the 2 DB nodes. DB node ILOMs are discovered, but not the DB nodes.
The error message in the Console UI pop-up screen says:
The following errors are found during discovery. Please examine the error messages and the targets being discovered if any.
<####dbnn.####.com : Infiniband Discovery Error>DiscoveryWarning: ibnetdiscover returns status 1. Please check the Infiniband switch hostname, nm2user password, and the Infiniband switch status.
<ORACLE_DBMACHINE>DiscoveryWarning: expect: spawn id exp5 not open

SSH connectivity works fine from all DB nodes to all 3 IB swtiches. We are able to connect without any password prompts whatsoever.
