EM 12c, EM 13c : Subscribing to Email From Incident Rules page Does not Send Page Notifications (Doc ID 1511555.1)

After defining the Page type Email Address for an Administrator in:  Click on the Administrators name on the top right of the console > Enterprise Manager Password and Email > Select: Add Another Row > Enter the address for the pager > Set Email Type to: Pager (Short)

While logged into the console as this Admin, subscribe this Administrator to an existing Incident Rule:  Setup > Incidents > Incident Rules > Click on the Incident Rule of interest > Click on Actions > E-mail > Subscribe me

Although subscribed to the incident rule, no pager notifications are sent by this rule.
