Cannot Login To FMWControl(FMWC) - Farm Name Missing on Login Page (Doc ID 1524396.1)

Symptoms

Unable to login to Oracle Enterprise Manager Fusion Middleware Control(FMWC).

Login Page is displayed without the farm name just above the username field.

After entering the Login credentials and click on login it never loads the subsequent pages.

Following errors are seen in the emoms.log and emoms.trc files in the $DOMAIN_HOME/servers/AdminServer/sysman/log directory -
