RAC One Node Instance relocate/failover is not correctly updated in 12c Cloud Control console (Doc ID 1548370.1)

Symptoms

Cloud Control is not automatically updating the monitoring configuration for a RAC One configuration on instance failover.

In RAC One there is only one instance running at any given time. In a failover/relocation situation the instance name will be started on another node, with a different instance name. The Cloud Control should automatically detect this scenario and update the monitoring configuration for the RAC Database with the correct instance name and the node it is now located on. In this situation the instance was relocated/failed over to another cluster node, but the Cloud Control was still showing the original instance name and on the wrong cluster node.
