EM12c: Log File Monitoring Only Logs First Occurrence Of String: Corrective Action Performed Only Once

Symptoms
On a WebLogic server target, set a Critical threshold and Corrective Action for Log File Monitoring -> Log File Pattern Matched Line Count.
An incident is only generated for the first occurrence of the match string in the log, and the Corrective Action is only executed once.
Set up the threshold and corrective action for the same log file on the Host target's Log File Monitoring metric and it generated incidents and executes corrective actions each time the match string is added to the log, as expected.
Additionally, the single incident on the Weblogic target does not appear under the metric name on the target's All Metrics page.
