EM 13c2 : Target Status Shows "Availability Evaluation Error" After Database Target Upgrade (Doc ID 2507084.1)

Symptoms

After upgrading database target from 12.2 version to 18.4, Target is marked as "Availability Evaluation Error" in EM console.
