EM 13c: Enterprise Manager 13c Cloud Control Shows Agent Status as Unreachable Despite the Agent Running and Uploading Correctly

Symptoms
Agent Version : 12.1.0.4.0 or later
OMS Version : 13.2.0.0.0
EM Cloud Control Shows the Agent Status as Unreachable despite the Agent is Running and Uploading Correctly.
EM console shows Upload Metric Data button greyed out.
Re-secure agent command works fine.
Connectivity test manually from OMS machine to Agent machine using telnet and wget works fine and there is no issue seen with respect to connectivity.
