EM 13c - Fleet Maintenance: Deploying RDBMS/GI Home Failed At Step "Setup Oracle Home Lineage"

Symptoms
In Enterprise Manager (EM) Cloud Control, When deploying new Oracle Home software it fails with below error:
$ emcli db_software_maintenance -performOperation -name=DEPLOY_NEW_HOME -purpose=DEPLOY_DB_SOFTWARE -target_type=oracle_database -target_list=<TARGET DB NAME> -normal_credential=<Credential_name:Credential_Owner> -privilege_credential=<Credential_name:Credential_Owner> -input_file=data:<Path to Input_scripts/<script_name>.txt>
Execution of above command failed at the step: "Setup Oracle Home Lineage" with the below error:
