EM 13c: Newly Added Targets Stuck in Status Pending (Target Addition In Progress)

Symptoms
From EM Console, newly added targets are stuck in Status Pending (Target Addition In Progress).
This issue is not specific to one target type. This can happen to any new target after discovering it in EM.
During this time, $../gc_inst/em/EMGC_OMS1/sysman/log/emoms_pbs.trc file reports following errors repeatedly:

java.sql.SQLException: ORA-14400: inserted partition key does not map to any partition
