Symptoms
Running SQL Tuning advisor on a database where non-default NLS settings are used may result in the wrong indexes to be created (ie. the indexes are already created).
Example of NLS parameters being set in the target database:

nls_sort='UCA0700_DUCET_S3_VN_BN_NY_EN_FN_HN_DN_MN'
nls_comp=LINGUISTIC
