13.5: OMS Upgrade Fails During OMSCA With Error: WSM-15026 : Failed to create the document in the repository (Doc ID 2782134.1)

Symptoms

Upgrading OMS to 13.5 fails during the OMSCA with the error:

OMS_HOME/cfgtoollogs/omsca/omsca_<timestamp>.log

Killing server EMGC_ADMINSERVER ...
Successfully killed server EMGC_ADMINSERVER
Killing OHS instance ohs1
Killing server ohs1 ...
This Exception occurred at Fri Apr 16 18:05:25 CEST 2021.
weblogic.nodemanager.NMException: Received error message from Node Manager Server: [OHS Server 'ohs1' is not currently running]. Please check Node Manager log for details.
Ignoring exception when stopping the OHS: main.WLSTException value: Error occurred while performing nmKill : Error killing server ohs1 : Received error message from Node Manager Server: [OHS Server 'ohs1' is not currently running]. Please check Node Manager log for details.
