EM13c : DB Discovery Not Appending "_<Hostname+FQDN>"

Symptoms
In Enterprise Manager (EM) Cloud Control, trying to discover the Primary & Standby Databases.
When they do discovery using the OEM console guided process, the target names are filled in correctly for the containers and pdbs, default target name for the database container and for the PDBS:
<CONTAINER NAME>_<Hostname+FQDN>
<CONTAINER NAME>_<Hostname+FQDN> _<PDB Name>
On the Primary DB instance, it fails to find the DB parameter file.
Due to which a default target name by appending the host name is being constructed.
i.e. "<DBSID>_<HostName>" which is not the case for standby instance.
