Symptoms

 Fleet UPDATE_SIHA completes successfully, but fails to mount asm disk with the following error

$ emcli db_software_maintenance -performOperation -name="update SIHA" -purpose=UPDATE_SIHA -target_type="has" -target_list="has_<target_name>" -normal_credential="<Cred_name>:SYSMAN"
-privilege_credential="<Cred_Name>:SYSMAN"

ORA-59066: Multiple disk paths (/dev/sde, /dev/oracleafd/disks/flash1) are discovering disk with identical disk header contents.
ORA-59066: Multiple disk paths (/dev/sdd, /dev/oracleafd/disks/data1) are discovering disk with identical disk header contents.
sqlStmt:ALTER DISKGROUP FLASH MOUNT /* asm agent *//* {0:0:128} */
CRS-5017: The resource action "ora.FLASH.dg start" encountered the following error:
ORA-15032: not all alterations performed
ORA-15017: diskgroup "FLASH" cannot be mounted
ORA-15040: diskgroup is incomplete
