When Using Financial Reporting (FR) Studio, The "Respond To Prompt" Dialog Does Not Display Properly (Doc ID 1615162.1)

Symptoms

You've launched Financial Reporting Studio and created or opened a report that contains at least one prompt.  When you launch the report and select the lookup icon for a prompt, the resulting dialog's graphics are incomplete.  Some icons may be replaced with a red X, the Available and/or Selected boxes might not appear, the layout of items might be incorrect and difficult to read, etc.
