Exalytics and OBIEE 11g: OBIEE 220.127.116.11.5 Login Fails with Error 500--Internal Server Error

After configuring multiple MSAD providers, the OBIEE login screen displays the following error, even though all OBIEE services are running:
Error 500--Internal Server Error

10.5.1. 500 Internal Server Error
The Server encountered an unexpected condition which prevented it from fulfilling the request.

1. New install of OBIEE 188.8.131.52.5 release
2. Added a new provider, MS Active Directory, and completed all configurations steps.
3. Successfully refreshed all guids.
4. Interim Patch for <Patch 21895214> MERGE REQUEST ON TOP OF 184.108.40.206.0 FOR BUGS 20188679 21382568 (Patch) was applied.
