Some Users Are Unable To Login To EPM Workspace Due To An Error: "Loading the module 'wksp.mode' failed"

Symptoms
1. Some users (native or external) are unable to login to Workspace. Others are unaffected. The login hangs at "Authenticating user" for a long time and then throws the error: Loading the module 'wksp.mode' failed
Upon clicking OK on that error and then "Show Details", the following is seen:

2. The Foundation Services Managed Server keeps restarting every few minutes. The Event Viewer had the following application error:
