Launching Discoverer Plus Via Forms Web.Show_document Causes Jinitiator Fatal Error (Doc ID 283928.1)

Symptoms

 An attempt to launch Oracle Discoverer Plus from Oracle Forms via
WEB.SHOW_DOCUMENT ('the discoverer url', '_blank')
is failing with the error:

JInitiator 1.3: Fatal Error - Attachment to a running java virtual machine failed

The discoverer authentication page is displayed, but after supplying the necessary log on credentials, the discoverer application window is launched and the Jinitiator error appears
