Essbase: Essbase Application Crashing with xcp File and Hangs the Essbase Agent (Doc ID 2908629.1)

Symptoms

An Essbase application crashes with an xcp file appear to also hang the Essbase agent where logins from MaxL and/or Smart View hang.

BSO application logs have the following error:

Fatal error [1130622] encountered during transaction commit.

ASO application logs have the following error:

1270082 Failed to flush file [D:\Oracle\Middleware\user_projects\<epmsystem-1>\EssbaseServer\essbaseserver1\APP\\<appname>\log\ess00001.dat]: a system file error occurred. Please see application log for details  ---- This error is an operating system error.
A system error occurred with error number [121]: [The semaphore timeout period has expired.]
Fatal Error(1270024) Fatal error at [aso/ukr/ukrbuffermanager.cpp], line [949]; terminating the server
