E1: FORMEXTNS: Form Extension Manager Displays "Reserved by <USER ID>"

Symptoms
In JPD920 environment on the web client, when user runs an application (e.g. P5503012A), after clicking on Form Extension icon, the error "Reserved by <USER ID>" is displayed in Form Extension Manager.
Steps to duplicate:
- Sign on to JPD920 environment on the web client
- Run the application (e.g. P5503012A)
- Click on the Form Extension icon
- Form Extension Manager is shown on the right
- The above "Reserved by <USER ID>" is seen in the Form Extension Manager
The form extension is being reserved by a user that is no longer in the company. In JPD920 environment, running the Work With User Defined Objects application (P98220U), found that the form extension object is in 02-Editing Status created by the user id. After deleting the user's Form Extension 02-Editing record in P98220 in PD environment, the error "Reserved by <USER ID>" is still there.
How to remove the reservation lock on the Form Extension with this user id?
Also, when user tries to promote the Form Extension from PY to PD environment, the user is unable to promote it as this form extension is being reserved by another user.
