Symptoms

The P54HS00 - Case number changes after each edit.  
Once assigned, we would like the case number to remain the same even if the incident is updated later on.

STEPS TO REPRODUCE
==================
1. P54HS00 add a new incident. Flag the Occupational Safety and Health Administration (OSHA) Reportable and (OSHA) Recordable and add Dr name.
Save the incident. Notice Case Number is updated with 1
2. Change the Dr. Name, and save the change.
3. Open the incident, Case Number changed to 2
4. Change a different value, this time Part of Body.
5. From Default to Multiple Body Parts, save the change.
6. Review the incident, Case Number now shows 3
