Run Health Check fails as follows:

Instance State: RUNNING
Login : success
Form Service App: failed
Form Service UBE: failed
JWT Trust Login: failed
Basic Auth: failed

AIS weblogic .out log shows the following error:

The JAS managed instance and the AIS managed instance are on the same JVM and both the instances' name starts with the letter U.
