Symptoms

When printing the Detailed Incident Report (R54HS400) all the check boxes were printed as ? instead of a checkbox.

Steps to duplicate:
1. P54HS00 lists Incident Numbers 45 and 46
2. From BV submitted R54HS400 - XJDE0001 and/or P54HS00 use the Print Report option.
3. The output RD54HS400 Detailed Incident Report shows the ? for some sections.
