Symptoms

Custom workflow is stuck in Awaiting status (status 07 in F98860) without any approval message being sent to the approver mailbox.
The same workflow instance is resumed and works fine if from P98860 the instance is selected and it is resumed from Row Exit -> Resume.

At the time when the issue was noticed, no steps to replicate the issue were discovered or known.
