 This document addresses various issues that may occur when using the Enrollment with Eligibility application (P08334).

Troubleshooting Steps
 Issue 1:  When a change enrollment is performed on the P08334, the limit on the Employee DBA Instructions is erased.
 Issue 2:  Enrollment with Eligibility (P08334) is not writing to the Employee DBA Instructions for Plan Options.
 Issue 3: When performing a Find, the Ok, Submit, and Retest Eligibility buttons are removed.
 Issue 4: A new plan is setup with plan options. The Enter Amount/Rate box is checked but when electing that benefit in Enrollment with Eligibility, the amount/rate box did not pop up.
 Issue 5: What controls the benefit and DBA end date for a terminated employee when using Form or Row exit Stop-Auto Dt/St?
 Issue 6: Employee who is enrolled in Employee Only and needs to be in Employee + Child and has payroll history, when enrolling through Enrollment with Eligibilty P08334 the system end dates the plan and the DBA correctly if ending the plan by itself. But when you enroll the employee in the new plan, it writes the enrollment, but not the DBA Instructions.
