OID ldap Operation (ldapadd, ldapmodify) Fails with "ldap_add: DSA is unwilling to perform" (Doc ID 1187023.1)

Oracle Internet Directory (OID) 10g or 11g.

Scenario 1:

An ldapadd fails with:

ldapadd -D cn=orcladmin -w <PASSWORD> -f myldiffile.ldif
adding new entry cn=<USERNAME>,cn=users,dc=<COMPANY NAME>,dc=com
ldap_add: DSA is unwilling to perform
ldap_add: additional info: Active Start and End Dates must be in UTC format (YYYYMMDDHHMMSSz) without any dashes, hyphens and spaces.

With the exception of the last line, this same error can occur with other versions of OID.
