When Attaching Content "From Repository" in Managed Attachments, No Select Checkboxes Appear (Doc ID 1275219.1)

Symptoms

When clicking attach "from repository", user can search for content, but no column of checkboxes appear to select content.
1. In E-Business Suite, open Zoom -> Managed Attachments 
2. Click the button "From repository". 
3. Search for content. 
4. Search results display, but no checkboxes appear.  The search results page is missing the left hand column of select checkboxes.
