OIDDAS 10.1.4.3.0 "Forgot Your Password?" Option Returns "Authentication Failed" For A Valid User

OIDDAS "Forgot Your Password?" option returns "Error: Authentication Failed" for a valid user
After adding additonal Password Reset Validation, now unable to get past Step 1 (Confirm Identity). The Delegated Administrative Services (OIDDAS / DAS) application returns the following message even though have entries/attributes for all password validation questions ok:
