Multiple Errors When Accessing the Imaging AXF Task List From EBS and Driver Page

Symptoms
When accessing the AXF task list from the EBS zoom menu, and from the AXF driver page (http://host:port/imaging/faces/Driver.jspx) the task list opens but the following error is displayed:
String Index Out Of Range: -1
The following errors can be seen in the Imaging diagnostic logs, depending on the Imaging version and Patch level:

Failed to load the provider: oracle.imaging.axf.view.metadata.AxfMetaDataService
Failed to load the provider: oracle.imaging.axf.model.bpel.BpelTaskSession
