Upgrade Assistant Shows ODI Already Upgraded Though ODI Studio Reports ODI-26168 Repostiory Errors (Doc ID 1329846.1)

Symptoms

You have been trying to upgrade ODI, in this example, from Oracle Data Integrator (ODI) 10g to latest ODI 11.1.1.5.

At step 6 of the Upgrade Assistant (UA) Examining Components, it shows already upgraded for Oracle Data Integrator:

At step 8 of Upgrade Assistant (UA) Upgrading Components, it shows Upgrading Progress 100% and for both Schema Version Registry and Oracle Data Integrator status, already upgraded.

However, when trying to log in from ODI Studio, you receive an error that looks like the one below (note the version numbers vary depending on the ODI versions):
