ODI Upgrade Fails with 'Target Work Repository upgrade failed.... Validation Query Failed' (Doc ID 1329880.1)

Symptoms

 You are running the Upgrade Assistant (UA) to upgrade from one version of ODI (in this case it is ODI 10.1.3.5) to ODI 11g, the upgrade fails.

In the UA log you see the following error:

 Best practices Regarding ODI Upgrade
 Can directly upgrade ODI 10g or ODI 11g earlier versions to the later ODI 11g versions for example ODI 11.1.1.5 or ODI 11.1.1.6.0 with no intermediate upgrade operations.
 Oracle recommends to upgrade cloned repository schema not the actual work repository schema, what is the reason behind it ? Can we just take a backup of the repository schema and perform the upgrade on the original schema?
 DEV environment is on ODI 11.1.1.5.0, can I only upgrade QA, the new environment I am creating to ODI 11.1.6.0?
 When upgrading a cloned repository schema to ODI 11.1.1.5.0 and later (11.1.1.6.0 etc), the UA persists in pointing to the original Work Repositories schema...
