Error Committing Batch "Error: 30028", Document Indexes are Clearing (Doc ID 1338940.1)

Symptoms

The following error is sometimes encountered when committing a batch from Index:

Error: 30028
The following error occurred while attempting to commit the batch:
[Batch failed to commit because a required field does not have an index value on page 1. Index name is Batch Origin.]

After the error, all index values are cleared out for page 1 and some other pages (e.g. pages 1-3 are cleared, but pages 4-<end of batch> are not cleared).
