PSA Command Fails With A SchemaUpdate Error While Trying To Patch A FMW Schema (Doc ID 1364170.1)

Symptoms

When applying the Patch Set to any installation type - the psa script needs to be run to upgrade the corresponding schemas, as per the Oracle Fusion Middleware Patching Guide 11g - 4 Updating Your Schemas with Patch Set Assistant

While running the $ORACLE_HOME/bin/psa script the following errors are logged in the $ORACLE_HOME/upgrade/logs/psa<timestamp>.log file and the command is not successful:
