HTTP 404 Error Accessing A Protected Resource on OHS Server Acting as a WLS Proxy in front of OAM

Symptoms
Oracle Access Server is installed following the Enterprise Deployment Guide. Webgate 11g is installed on the OHS server containing the mod_wl_ohs proxy in front of OAM.
Accessing the protected resource fails with a HTTP 404 error for a request to "/oam/server/obrarreq.cgi".
HTTP header trace shows:
