SOA 11g: Older, Previously Deployed Code is Loaded by the SOA Managed Server (Doc ID 1379771.1)

Symptoms

After restarting the SOA managed servers older deployments are intermittently loaded rather then current deployments. 
Upon export from MDS, the deployed-composits.xml shows corrupted entries for the offending app. An app should only have one entry along the lines of:
