Symptoms
Scanning and committing color or grayscale TIF LZW compression images in Capture or Distributed Capture; and committing as "PDF - Image Only" for the Document Output Format; and using the "Generate PDF/A documents" option will sometimes generate all black PDF files instead of the correct representation of the images.
