Symptoms

Need To Know (NTK) prevents access to the file if it is the first revision and in the workflow.
NTK tries to get security info from the latest released, which doesn't exist.
3 servers, one with CS10gR35CoreUpdate build 37 - working.
2 with CS10gR35CoreUpdate build 42 - experiencing the issue.
Same version on NTK on all 3 servers, as well as other relevant components
