Can't start a new publish, or delete an existing publish destination (Doc ID 1402234.1)

Symptoms

Why am I unable to start a publish, or delete the publish destination. I receive the message:
A publish session is already running for this destination. Please check the Publish console and try again when that publish operation is complete.

The following error may been seen in futuretense.txt / sites.log

[fatwire.logging.cs.xcelerate] [pagename=OpenMarket/Xcelerate/Actions/EventPublish] Error: com.openmarket.xcelerate.commands.PubSessionManagerDispatcher: Create: java.lang.NullPointerException
