File Upload from JSP with "multipart/form-data" Does Not Work on WebLogic 12.1.1.0 (Doc ID 1452409.1)

Symptoms

Environment Details:

WLS 12.1.1
OS: Microsoft Windows 7 (32-bit)

Issue:

File upload from JSP with "multipart/form-data" does not work on WebLogic 12.1.1.

When the application runs on WebLogic 12c, all the form's value in JSP page are lost once submitted which used to work in earlier version of WebLogic Server 11g.
