Unable to Check-in Documents due to Unique Constraint Error

Users cannot check-in documents. This includes uploading new documents, or uploading revisions of existing documents. The error both in the browser and occurs in the logs/system audit is 'Violation of PRIMARY KEY constraint 'PK___3__10'. Cannot insert duplicate key in object 'dbo.Documents'.'

Steps to Reproduce
1. Check-in document from Content Server web interface


Impact on Business
Policy Managers cannot upload new policy nor upload revisions for statewide public and internal use.
