OAM 11g: Windows Native Authentication Fails With "An Incorrect Username Or Password Specified" - KVNO Miss Match (Doc ID 1460606.1)

Symptoms

 When attempting to login via WNA, users receive the error "An Incorrect Username Or Password Specified".  In the OAM log, we see the following error message (java stack truncated):
