AD Connector 11.1.1.5 Fails with IntegrationException: Connector ConnectorKey not found

Symptoms
Installed AD 11.1.1.5 Connector and Connector Server 11.1.1.5 is up and running.
When trying to run Active Directory Group Lookup Recon / Organization Lookup Recon or trying to provision AD User, the following exception is get: oracle.iam.connectors.icfcommon.exceptions.IntegrationException: Connector ConnectorKey( bundleName=ActiveDirectory.Connector bundleVersion=1.1.0.6380 connectorName=Org.IdentityConnectors.ActiveDirectory.ActiveDirectoryConnector ) not found.
For example, when trying to provision AD User, log file shows:
