Forms Page Access Results In 404 Error / formsapp.ear missing (Doc ID 1469718.1)

Symptoms

Accessing:

 

All URLs used to access a Forms application result in 'HTTP 404 Not Found'


http://<IP ADDRESS>:port/forms/frmservlet
http://<IP ADDRESS>:port/forms/frmservlet
http://<HOSTNAME>:port/forms/frmservlet
http://<HOSTNAME>:port/forms/frmservlet

===> errors out with HTTP 404 Not Found

Exact error message:

Error 404--Not Found From RFC 2068 Hypertext Transfer Protocol -- HTTP/1.1:10.4.5 404 Not Found


http://<IP ADDRESS>:port--> works as expected (welcome page for HTTP is shown)


WLS_Forms is up and running in the AdminConsole
Node manager is also started
HTTP server is up and running verified by opmnctl status -l
