Symptoms

Configuration:

WebCenter 11.1.1.5.4 (Bundle Patch 4).

Active Directory (AD) has been configured as identity provider, and principle user write access not granted to AD users.
Use Case: 

After applying 11.1.1.5 BP4 an uploaded photo in the profile page does not display.
ERROR found in WebCenter log:
