Symptoms
The Forms Java Applet (or any Java Applet) no longer runs on any web browser on the OSX system.
You will see the following inside the browser:

The JRE was updated to JRE 1.6.0_37 either using:
10.6.8 - Java for Mac OS X 10.6 Update 11
or
10.7.X and 10.8.X - Java for OS X 2012-006
